Office Policies
Because mutual understanding is the basis for good relationships, it is important for you to understand the nature of our office policies. We look forward to building a great relationship with you. Welcome!
Appointments:

Being on time for appointments is crucial. If a child arrives 15 minutes or more past the appointment time, they will not be seen.

If you need to reschedule, please give at least 2 business days' notice. Missed appointments will result in a $50 fee.

Missed appointments during prime time or no school days will not be offered again.

Two missed appointments will result in the dismissal of the entire family from the practice.

If you schedule multiple children on one day and miss the appointment, the practice reserves the right to schedule family members separately.

Appointments must be confirmed at least 2 business days in advance, otherwise, they will be automatically canceled.

If someone other than the parent is bringing a child to their dental appointment, the practice needs at least 3 hours' notice in advance to email the authorization form.

A parent must stay with their child throughout the visit. Additional children must remain seated next to the parent or carried.
Insurance:

Insurance information must be provided at the time the appointment is scheduled.

If you are an existing patient, any new insurance information must be provided at least 48 hours in advance to allow enough time for verification.

The practice reserves the right to deny services if insurance information has not been provided 48 hours in advance.

It is the patient's responsibility to understand what their insurance covers.

After 30 days of an outstanding balance, an additional 10% will be charged. After 60 days, it will be 20%. After 90 days, legal fees may be added to the balance, and/or the account will be sent to collections.
Financial Responsibility:

Payment is expected before or at the time of service.

If behavior techniques are necessary to complete treatment, there will be a behavior management fee of $108 charged to the parent.

After multiple behavior management attempts, a cost of $50 will be charged to cover time and supplies.
The practice requests that patients and their families understand and follow these policies. If there are any questions, comments, or concerns, patients are encouraged to contact the practice as soon as possible.
